הפיגוע התרחש בסמוך לאחת מתחנות הרכבת הקלה ביפו. שני המחבלים הם מוחמד חלף סאהר רג'ב וחסן מוחמד חסן תמימי, שוהים בלתי חוקיים בשנות העשרים לחייהם ותושבי העיר חברון. על פי החשד, שני המחבלים החלו במסע הרצח כשהם נכנסו לקרון של הרכבת הקלה שעצרה בתחנה וביצעו ירי לעבר הנוסעים. לאחר מכן הם המשיכו רגלית על רחוב שד' ירושלים תוך שהם פוגעים בעוברי אורח. המחבלים נוטרלו על ידי סיירי סל"ע של עיריית ת"א-יפו ואזרחים שהיו במקום.
לפי פרטי החקירה הראשוניים, המחבלים התפללו באחד המסגדים, ואחר כך אמרו לנוכחים במסגד לא לצאת מהמקום. הם דקרו חייל ברכבת הקלה וחטפו את נשקו. המחבלים ירו בעוברים ובשבים עד ששוטרים וסיירי הביטחון של עיריית תל אביב נטרלו אותם.
האמאם של המסגד בו התפללו הרוצחים המתועבים לתקשורת הישראלית כי לדבריו : "אף אחד מאיתנו לא ידע על כוונת המחבלים. ברגע ששמענו ירי היינו הראשונים להתקשר למשטרה. אנשים לא אחראים יכולים להדליק את כל הגזרה הזו שהיא שקטה מ-7 באוקטובר".
